The first MIETHKE valve

FUNCTIONALITY

The DUALSWITCH-VALVE is the first MIETHKE valve. It was first implanted in 1995 at the Charité hospital in Berlin and quickly achieved very good results in clinical studies.

The DUALSWITCH-VALVE consists of two parallel valve chambers, one for the lying and one for the sitting or standing body position. Due to this unique design, the valve automatically detects whether the patient is in a lying or upright position and thus always sets the correct valve opening pressure. This effectively prevents unintentional and also dangerous overdrainage.

Horizontal Position

If the intracranial pressure is within its physiological limits, the valve remains closed. If the intracranial pressure rises, the low-pressure chamber of the valve opens and prevents a further increase in intracranial pressure through the now possible drainage of cerebral fluid.

Vertical Position

If the patient moves to the upright body position, the low pressure chamber of the DUALSWITCH-VALVE is closed by the tantalum ball. Hence, only by way of the high pressure chamber is CSF-drainage possible. The opening pressure in the high pressure chamber is higher than the opening pressure in the low pressure chamber. By this mechanism the DUALSWITCH-VALVE prevents overdrainage in vertical body position.

Each preset pressure stage is specially coded and can be recognized postoperatively in imaging. The lower illustrations show the X-ray images of all available pressure stages of the DSV (5/30, 5/40, 5/50, 10/30, 10/40, 10/50 and 13/30, 13/40, 13/50 cmH2O:

The MIETHKE App shows clearly and quickly understandable how our valves work:

+ Influence of the body position on the opening pressure: The motion sensor of the smartphone simulates the body position of the patient: upright - inclined - lying.

+ Pressure stage recognition in the X-ray image: Every set or preset pressure stage is shown here and can be easily recognized by means of templates.

+ Simulation of adjustability: Valves can be easily adjusted with the finger and thus help to understand what is happening inside the valve.

+ View into the inside of MIETHKE valves: All valves allow a view of their construction and thus the influence of the cerebrospinal fluid flow on the functioning can be shown in an easily understandable way.
